Records Request

Private Records

Private records are records with personally identifiable information about a client. Private records can only be requested by the individual or the legal guardian of a child under 18 years of age.

Birth Certificate
Birth certificates can be ordered online, by mail, fax and in person. Visit the birth certificates webpage for more information.

Death Certificate
Death certificates can be ordered online, by mail, fax and in person. Visit the death certificates webpage for more information.

Foodborne Illness Investigation Record
To request a foodborne illness investigation record on a complaint you previously filed with the health district, download the Authorization to Disclose Patient Health Information. PDF

The form may be mailed, faxed or emailed to legal@snhd.org.

Note: the form must be signed with an original signature, not signed electronically.

Immunization Record
To download immunization records for yourself or a child under 18 years of age for whom you are the legal guardian, visit the Nevada WebIZ Public Access Portal.

If unsuccessful, contact the State at: izit@health.nv.gov or (775) 684-5954. Once you have spoken with someone at the State you will be able to download your records immediately via the State of Nevada’s WebIZ Public Access Portal.

Note: If you choose to email the State, be sure to include: the legal name, DOB, your association, if requesting on behalf of a minor and a phone number or email address you would like the State to use for the records. Should you decide to call, the State’s staff members are working remotely; therefore, you will have to leave a voicemail.

Medical Record
To request medical records for yourself or a child under 18 years of age for whom you are the legal guardian, download the Authorization to Disclose Patient Health Information in English or Spanish. PDF

Note: For COVID-19 test results, please contact the medical provider that issued the test. The Southern Nevada Health District can only provide test results for those who were tested at a Health District facility or by Health District staff.

Morbidity Report
To request a morbidity report for yourself or a child under 18 years of age for whom you are the legal guardian, download the Authorization to Disclose Patient Health Information in English or Spanish PDF

Public Records

Public records include inspections of permitted facilities, statistics, meeting minutes, etc.

Board of Health Records
Visit the Board of Health webpage for agendas and minutes from 1996 to present, supporting documentation from Sep. 2007 to present, and audio recordings from Dec. 2010 to present. If you are unable to locate what you are looking for, complete a Public Records Request Form.

General Public Records
Complete a Public Records Request Form if you need a public record, such as restaurant, pool or hotel inspections.

Public Health Data
The health district collects and analyzes health-related data in Clark County. Visit the Southern Nevada Public Health Data Requests webpage for more information about the types of public data available.

Solid Waste Records
