“What is Linkage to Action?”

SNHD’s Linkage to Action (L2A) team is funded through the Centers for Disease Control and Prevention’s (CDC) Overdose Data to Action (OD2A) grant. L2A is dedicated to preventing fatal and non-fatal overdoses through education, surveillance, and linkage to supportive services. The team is housed at the Clark County Detention Center which allows L2A to provide enhanced linkage to individuals with complex needs and who are at greater risk for overdose. Services provided include referrals to peer navigators, referrals to medically assisted treatment upon release, referrals to mental health providers and sober living if eligible and providing harm reduction education and supplies such as Fentanyl Test Strips (FTS) and naloxone which can be placed in the incarcerated person’s belongings and distributed at release.

L2A also operates out in the community through coordinated outreach activities with SNHD’s OD2A subgrantee partners who provide services and supplies not currently offered by L2A such as sterile syringes, wound care and direct peer support. The team’s mobile outreach unit allows the team to visit communities such as encampments, tunnels, and other areas that are disproportionately impacted by fatal and non-fatal overdose.
