Health Alert Network Enrollment Form
The Health Alert Network (HAN) is a nationwide communications system that was established by the Centers for Disease Control and Prevention, and is implemented by state and local health departments.
HAN is designed to promote 24/7 flow of critical health information between the State and Local Health Departments and local and rural health care professionals throughout the state. The health district maintains a HAN database of health care providers by specialty so important public health messages can be distributed relevant to their practice.
HAN messages range from important information about local and national disease transmission and product recalls to alerts which require immediate action. HAN members are identified and tracked in a secure database maintained by the Office of Epidemiology.
